Mid-module evaluations are a great way to get feedback on what the students are experiencing and can provide insights for tweaking the delivery of the course to satisfy the student's needs.
To create an evaluation form, in the section you want to add the form to, select Create Learning Activity, and choose Feedback from the activity grid.
Give the activity a title, and, using the options on the right-hand side of the setup page, select the Availability of the activity. The default setting is that responses are anonymous: this should not be changed and should be emphasised to students when instructing them to complete this.
Select Save and Display.
Now select Templates, and choose the appropriate template from the dropdown.
Do not make any alterations to this template. Select Save Changes to return to the activity page.
The module evaluation will now be available to students from the times selected.
